What is a tooth extraction?

A tooth extraction is the removal of a tooth from its socket in the jawbone. Extractions fall into two categories: simple and surgical. A simple extraction is performed on a fully erupted tooth. A surgical extraction is more complex and is for teeth that are not easily accessible such as removing an impacted wisdom tooth.

Common reasons an extraction is recommended include severe decay where the tooth cannot be reliably restored, a tooth fractured below the gum line, advanced gum disease causing significant bone loss, or an orthodontic indication. Where a gap is left after removal, Dr Maini will discuss replacement options, including implants, bridges or dentures, before any treatment begins.

Pre Treatment

At your first appointment, Dr Maini reviews your medical history, examines the tooth and takes any necessary radiographs. You will be told whether the extraction is simple or surgical, which anaesthetic option is appropriate, and what the plan is for the gap once the tooth is out. A clear written quote is provided before any treatment is agreed.

Treatment

The area around the tooth is fully numbed under your chosen anaesthetic. For a simple extraction, the tooth is loosened gently and removed; most patients describe the sensation as pressure rather than pain. For a surgical extraction, a small flap of gum is raised and the tooth is sectioned where needed to allow a clean, careful removal. If you have chosen sedation or general anaesthesia, this is administered at this appointment.

Post Treatment

Once the tooth is out, gauze is placed over the socket to support clotting. You will receive written aftercare instructions covering swelling, what to eat for the first 24 hours, and warning signs to watch for. Initial soft tissue healing typically takes a few days, with full healing within one to two weeks. A follow up appointment is offered where clinically indicated.

Side Effects and Risks

Common side effects after an extraction include mild swelling, bruising, or temporary soreness, managed with over the counter pain relief. There is a small risk of infection, dry socket, or delayed healing. Dr Maini takes every precaution to minimise these risks, and all potential complications are discussed with you in full before treatment is agreed.

Why Act on a Damaged or Infected Tooth?

Delaying treatment for a damaged or infected tooth can lead to worsening pain, abscess and the spread of infection to neighbouring tissues. That said, extraction is not always the right answer. Sometimes root canal treatment or restoration can save the tooth. Dr Karan Maini will assess your situation honestly and explain the options clearly before any treatment is agreed.

We understand that dental anxiety is the most common reason adults put off an extraction for months or even years. At Zenith, three anaesthetic options are available. Most extractions are performed under local anaesthetic alone; the area is fully numbed and you remain awake throughout. For nervous patients or more complex surgical cases, conscious IV sedation produces a deeply relaxed state where you are unlikely to remember the procedure clearly.

Where conscious sedation is not sufficient, general anaesthesia is available through Zenith's established pathway with a consultant anaesthetist present throughout. The right option for you is discussed openly at consultation, based on the clinical picture and your own preference.

Suitability for sedation and general anaesthesia is confirmed at consultation, including a pre anaesthetic medical review. Local anaesthetic remains the most common choice for straightforward simple extractions.

After Your Extraction. What Are Your Options?

If the tooth being removed will leave a gap that matters visually, functionally, or to protect your bite, Dr Maini will discuss your replacement options before the extraction takes place. Depending on your clinical situation, this could mean dental implants, a bridge, or dentures. Where an implant is planned, a bone graft at the time of extraction can preserve socket volume and make the implant procedure simpler later.

Will the tooth extraction be painful?

You may feel some pressure during the procedure, but a local anaesthetic is used to numb the area so you should not feel sharp pain. Most patients find the experience more comfortable than they expected. We will also provide clear written aftercare instructions to help manage any soreness afterwards.

When is a tooth extraction needed?

Tooth extraction is usually recommended for severe decay where the tooth cannot be reliably restored, a tooth fractured at the gum line, advanced gum disease, or an impacted wisdom tooth causing recurrent infection. It may also be needed for orthodontic reasons or where a previous root canal treatment has failed. Dr Maini will confirm at your consultation whether extraction is the right course of action for your case.

How long does it take to recover after a tooth extraction?

Initial soft tissue healing typically takes a few days, with full healing within one to two weeks. Bone remodelling beneath the gum continues for several months. We provide detailed written aftercare instructions at your appointment to support the healing process and help prevent complications.

What happens if I do not remove a damaged tooth?

A severely damaged or infected tooth that is left untreated can lead to ongoing pain, abscess, and infection spreading to neighbouring tissues and bone. That said, extraction is not always the right answer. Sometimes a root canal and crown can save the tooth. The decision is made at consultation based on a full clinical assessment.

Can I return to work the same day as my extraction?

After a simple extraction under local anaesthetic, many patients return to work the following day. After a surgical extraction, or where sedation or general anaesthesia has been used, plan for one to two days off depending on how you feel. We will give you a realistic estimate based on your specific procedure at your consultation.

Can I drive home after my extraction?

If your extraction is carried out under local anaesthetic alone, yes. If you are having IV conscious sedation or general anaesthesia, you must not drive afterwards. A responsible adult will be required to take you home and ideally stay with you for the rest of the day.

What is a bone graft and will I need one?

A bone graft, sometimes called socket preservation, is a small procedure carried out at the time of extraction to maintain bone volume in the socket. It is typically recommended where a dental implant is planned at the same site in the future. Preserving the bone now makes the implant procedure more straightforward later. Dr Maini will advise you at your consultation whether it is appropriate for your case.

How much does a tooth extraction cost?

A simple extraction at Zenith starts from £217. Surgical extractions, such as impacted wisdom teeth, are quoted individually after clinical assessment and radiograph. If you choose IV conscious sedation or general anaesthesia, this is quoted separately. A clear written quote is provided before any treatment is agreed, with no pressure and no obligation.
