What is Guided Biofilm Therapy?

Guided Biofilm Therapy, or GBT, is a modern, structured protocol for professional dental cleaning, developed with the Swiss EMS group and now used in well equipped dental practices across the UK and Europe. Where a traditional scale and polish removes calculus first, GBT does the opposite. It makes biofilm visible, removes it gently and thoroughly with warm water air polishing, and only then uses ultrasonic instrumentation where it is actually needed.

That order matters. Biofilm, the soft bacterial film that coats teeth and gums every day, is the underlying cause of gum disease, decay and bad breath. Calculus (tartar) is biofilm that has hardened. Removing biofilm first with fine erythritol powder rather than abrasive paste addresses the actual problem at its source. The ultrasonic step at the end becomes shorter, gentler and more focused, because most of the work has already been done.

Pre Treatment

We review your medical history and any concerns since your last visit, then carry out a brief clinical examination of your teeth, gums and any existing restorations or implants. A small amount of harmless disclosing dye is applied to make biofilm clearly visible to both you and your clinician, so we can show you exactly where bacteria is building up before treatment begins.

Treatment

Fine erythritol powder in warm water is used to remove biofilm, early calculus and surface stain across all tooth surfaces, including the spaces between teeth. Where pockets are present, a specialised PERIOFLOW nozzle reaches gently beneath the gumline. Where calculus has hardened after air polishing, a fine piezoelectric ultrasonic tip is used to remove it precisely. Treatment usually takes 30 to 45 minutes.

Post Treatment

After your GBT appointment, your teeth will feel noticeably cleaner and smoother. A final visual check confirms that biofilm has been thoroughly removed. We then discuss a recommended recall interval based on your specific risk profile and home care routine. You can eat and drink as normal straight away, though avoiding very hot or very cold drinks for a few hours may help if you notice any residual sensitivity.

Side Effects and Risks

In published randomised controlled trials, GBT is rated as more comfortable than traditional scaling, with patients reporting less sensitivity and clearly preferring the protocol, while clinical outcomes for plaque, bleeding and pocket reduction are comparable or better. Most patients describe the experience as noticeably different from a conventional hygiene visit. For patients who have put off attending due to previous discomfort, that comparison matters.

GBT removes surface stain very effectively and often gives a visibly brighter result, but a scale and polish is not a whitening treatment. If you want to lift the underlying shade of your teeth, we will discuss teeth whitening separately rather than overselling what cleaning can achieve.

How often should I have a GBT appointment?

For most adults, every six months is the right interval. Patients in active periodontal care, smokers, patients with implants, or anyone with a history of heavy calculus build up may benefit from a three or four monthly recall. Your clinician will discuss the recommended interval at the end of your first visit, based on your specific risk profile and home care routine.

Is GBT suitable for nervous patients?

Yes. GBT is specifically more comfortable than traditional scaling. The warm water and erythritol air polishing steps are significantly gentler than conventional instrumentation, and the absence of cold water removes one of the most common sources of sensitivity. We are happy to discuss your concerns before your appointment, and our team will go at a pace that suits you.

What if my gums bleed during or after the appointment?

Some bleeding during the appointment is common, particularly if it has been a long time since your last hygiene visit or if you have any underlying gingivitis. It is a sign that the gums need attention, not a reason to avoid the appointment. Bleeding typically settles within a few days as the gum tissue heals, and consistent home cleaning between appointments is what keeps it from returning.

Can I eat and drink straight after my GBT appointment?

Yes. There is no restriction on eating or drinking after a GBT appointment. Some patients prefer to avoid very hot or very cold drinks for a few hours if there is any residual sensitivity, but this is not a clinical requirement. You can return to your normal routine straight away.

Will GBT damage my existing crowns, veneers or bonding?

No, quite the opposite. Erythritol air polishing is significantly less abrasive than the rotary brushes and prophy pastes used in traditional polishing, which over a lifetime of visits can dull or scratch the surfaces of crowns, veneers and composite restorations. GBT is the protocol we specifically recommend for patients with extensive restorative work.

Can I have GBT if I am pregnant?

Yes. GBT is safe in pregnancy, and good gum care during pregnancy is particularly important. Pregnancy gingivitis is common, and untreated gum inflammation has been linked in some studies to wider health outcomes. We routinely treat pregnant patients with the standard GBT protocol, and your clinician will review your medical history at the start of your appointment.

Will GBT make my teeth look whiter?

GBT removes surface stain very effectively and often gives a visibly brighter result. However, a scale and polish is not a whitening treatment, and we will not claim otherwise. If you want to lift the underlying shade of your teeth, our team will discuss professional teeth whitening with you as a separate treatment rather than overselling what cleaning can achieve.

How much does a scale and polish cost?

Your first GBT visit is priced at £112. Subsequent visits are £149. Patients with longstanding calculus or active gum disease may require more than a single visit, in which case a clear plan and quote is provided at the first appointment before any further work begins. The Zenith Dental Plan includes two GBT visits a year, which represents a significant saving against paying per visit. All prices include VAT.
